This is a 30th-anniversary pamphlet sold exclusively at the Cornelius 30th Anniversary Set live venue.
The included CD has been roughly cut out, so it is in the condition shown in the images (image 3).
Also, there is a slight fold on the corner as shown in the 4th image.
Other than that, there are no writings or markings.

The included CD was originally as shown in the images, and there was no printing on it. It will be shipped in a CD case as shown in the images (image 5).

The pamphlet itself has no noticeable scratches or stains, but please refrain from purchasing if you are sensitive to this, as it has been stored at home.
